This Privacy Policy describes how Fathom, Inc. and our subsidiaries and affiliates (collectively, "Fathom", "we", "us", or "our") collect, use, and share your personal information in connection with the www.fathomhealth.com website and our other websites and online services to which we post this Privacy Policy (collectively, the "Service"). On occasion, we may inform you that our use of your personal information is subject to a different privacy policy or different terms, in which case the different privacy policies or terms will apply instead of this Privacy Policy. This Privacy Policy does not apply to patient information or other personal information that we process on behalf of our business customers as part of the services we provide them ("Service Data"). We use Service Data only as authorized by our contracts with our business customers.

Personal information we collect

Information you provide to us. Personal information you may provide to us through the Service or otherwise includes:

  • Contact information, such as your first and last name, email and mailing addresses, phone number, professional title and company name.
  • Registration information, such as information that may be related to an event you register for.
  • Feedback or correspondence, such as information you provide when you contact us with questions, feedback, customer support requests, or other correspondence that you send us.

Information we obtain from social media platforms. We are headquartered in the United States and may use service providers in other countries. Your personal information may be transferred to the United States or other locations where privacy laws may not be as protective as those in your state, province, or country.

Children

The Service is not intended for use by children under 13 years of age. If we learn that we have collected personal information from a child under 13 without the consent of the child’s parent or guardian as required by law, we will delete it.
